In this Privacy Policy, ‘we’, ‘us’, and ‘our’ refers to Perth Radiological Clinic and all associated entities including PerthRadClinic Limited, Victoria Street Radiology, Joondalup Radiology and Magnetic Resonance Centre. The Privacy Act 1988 requires us to inform you of your rights relating to the collection, use and disclosure of your personal information.

We reserve the right to amend this Notice at any time and your use of the website following any amendments will represent your agreement to be bound by these terms and conditions as amended. We therefore recommend that each time you access our website you read this policy.

Collection

This website does not automatically capture or store personal information, other than logging the user’s IP address and session info such as the duration of the visit and the type of browser used. This is recognised by the web server and is only used for system administration and to provide statistics which are used to evaluate use of the site. We do not use cookies for collecting user information from the site.

Health Records and Information Privacy

Only you and the people you authorise can request access to your information. No other medical practitioner, hospital, health fund or research body can access that information.

Use and disclosure

We have a professional duty to maintain the confidentiality of your information, and we will not disclose information about you unless we have your instructions, or are under a legal obligation, to do so.

Our practice staff will use and disclose your information for purposes such as:

Other Medical Providers’ Access to your Medical Records

Access to your medical records, which includes reports and images, is provided to your referring doctor in the course of your care. On occasions, your referring doctor may request that your records are made available to another doctor for the purposes of second opinion or further management. In addition, your doctor may request that your images and reports are made available to another radiology provider to provide a further opinion, or for comparison/correlation with subsequent imaging. All access to PRC patient data provided to other clinicians, facilities or imaging providers is logged. This access is provided based on consent obtained from your referring doctor of clinicians personally involved in your current care.

Access Limitation Options

Access to patient records can be restricted in certain circumstances. These records are then only made available to your referring doctor for the specific test that they have requested. No further access is granted without your specific patient consent.

There are two categories:

VIP patients

VIP patients attending the clinic are logged as a special subset. There is limited access to view data internally and no ability to transfer cases beyond the referring clinician without an express written consent on each episode of care.

Access Denied Patients

You may elect to opt out of the process outlined above (which allows granting access to reports and imaging data amongst medical providers who are involved in your care). This process requires you to download and submit the “OPT OUT – Access Denied Patient form”. Patients can download the form from our website or request to fill out the form at our clinic.

What is the effect of OPTING OUT- Access Denied?

Your referring doctor will receive the data in the usual way, but PRC will NOT release access to your data to any other health care provider or facility without your express written consent. Please note that this may lead to significant delays or compromise of your own medical management, hence the implications for Opting Out- Denying Access should be considered carefully and discussed with your health practitioner.

Information Security and Storage

We take appropriate steps to ensure that all information we hold is protected from loss, misuse, or unauthorised access, disclosure or modification. We maintain physical security over our premises and access to our computer systems is limited by user identifiers and passwords.
